Introduction

In the UK guidance National Institute of Clinical Excellent (NICE) guidance [1] advised early referral for colonoscopy based on symptoms suggesting lower GI malignancy. In June 2022 updated national guidance [2] was published recommending the use of FIT for all community based referral for colonoscopy. A regional FIT testing strategy (cut off 10gHb/g faeces) was implemented from July 2022.

Aims & Methods

To evaluate the outcome of implementing a community based FIT strategy on a regional Health Board colonoscopy service, focussing on diagnostic rates for colorectal cancer (CRC). Data sources included laboratory based FIT registry, EMS endoscopy reporting system and Welsh Clinical Portal (pathology, laboratory, and imaging) for all patients with a community based FIT test between July-December 2022. CRC rates for this cohort were compared with 1) Index Bowel Screening Wales programme patients (aged between 58 and 74 years of age) (referred if FIT result >120 ugHb/g faeces) undergoing colonoscopy in the same time period [n=180], and 2) a historical cohort of symptomatic patients [n=191, 78M:113F, mean age 47.7years, range 18-92] referred for lower GI endoscopy (July-December 2021) when symptom-based referral criteria applied. Differences between the CRC detection rates between each cohort were tested using the Chi-squared test.

Results

A total of 3291 FIT tests were performed (Jul-Dec 2022), 162 were positive (FIT+ rate 4.9%) [89M,73F; mean age 65.7years (range 23-91)]. 48/162 (29.6%) were anaemic. Of the FIT+ cohort 107 underwent colonoscopy and 36 had CT colonography (21 did not complete definitive diagnostic testing). 17/162 (10.4%) had colorectal cancer, with a further 20 (12.3%) have advanced neoplasia. CRC rates increased with increased FIT values: 10-80, 4.8%; 80-120, 11.1%; 120-400, 18%; >400, 25.9%. 107 colonoscopies performed to screen the FIT test cohort detected 16 CRC (14.9% detection rate and were significantly higher than comparator groups attending for colonoscopy; 12/180 (6.7%) in the BSW group [Chi-squared=6.18, d.o.f.1, p<0.025], and 3/191 (1.6%) in the historical symptomatic cohort [Chi-squared=20.65, d.o.f. 1, p<0.001]. Clinically important incidental pathology was detected in a further 11 patients during the course of their investigative pathway.

Conclusion

A regional community FIT testing strategy has been successfully implemented and supported. This has increased significantly diagnostic yield of CRC at colonoscopy vs previous symptom-based referral and national screening programmes. These benefits must be offset against the additional costs of FIT testing, providing clinical oversight and pathway safety netting and managing incidental pathology detected.

Introduction

There are several reports about the diagnosis of early gastric cancers (EGCs) using artificial intelligence (AI) systems, but satisfactory data for sensitivity and specificity are not yet available. Linked color imaging (LCI) produces high color contrast between malignant lesions and surrounding mucosa compared to white light imaging (WLI). Therefore, LCI combined with AI (LCI-AI) has the potential to further improve the diagnosis of gastric cancer compared with WLI combined with AI (WLI-AI). We developed three new LCI-AI models to determine their sensitivity and specificity for the diagnosis of EGCs.

Aims & Methods

Three models including a convolutional neural network (CNN) model (high resolution net (HRNet)), a transformer model (Segformer) and a hybrid (combined CNN and transformer) model (Uniformer), were based on MMSegmentation. When each model recognized a gastric cancer lesion from test image data, it was shown with the existing position as a blue area on WLI images and green area on LCI images. Non-neoplastic mucosa was shown as a black area on both images. Endoscopic images using WLI and LCI (LASEREO 7000 system series and scopes of EG-L600ZW and EG-L600ZW7, Fujifilm Corporation, Tokyo, Japan) were collected from patients with EGCs from January 2015 to December 2018 at Jichi Medical University Hospital. A total of 1970 images including 1179 WLI and 791 LCI images of 236 EGCs were used for training the three LCI-AI models. Another set of 414 EGC images was selected for validation from 43 cases. Primary and secondary endpoints were the sensitivity and specificity of each AI model, respectively.
The image-based performance metric was based on Dice loss, calculated by subtracting the mean Dice similarity score. A Dice score >20% was defined as the correct diagnosis of cancer because EGCs sometimes have unclear demarcation lines. The correct diagnosis of a non-malignant lesion was defined as no reactive black areas in surrounding mucosae. Each case had from four to 21 images. Therefore, the median Dice score was used as a representative value in each case to evaluate performance. All three models’ programming and data analysis were supported by AI Medical Service Inc. (Tokyo, Japan).

Results

Analysis of lesions in all EGC images showed both sensitivity (82.8%) and specificity (83.4%) with LCI-AI to be high and similar to those with WLI-AI (83.1% and 84.3%, respectively). Sensitivities of LCI-AI/ WLI-AI among HRNet, SegFormer and UniFormer were 80.9%/81.6% (p=0.625), 83.8%/83.8% (p=0.224) and 83.8%/83.8% (p=0.450), respectively and specificities were 80.0%/86.0% (p=0.120), 86.4%/84.8% (p=0.672) and 86.8%82.0%/ (p=0.214), respectively. Representative Dice values among cases with EGCs showed sensitivities for LCI-AI using HRNet, SegFormer and UniFormer to be extremely high, at 92.7% 90.9% and 93.0%, respectively, although not significantly different when compared to those using WLI-AI, 84.6% (p=0.67), 85.4% (p=0.15) and 80.5% (p=0.19), respectively. Higher sensitivities were found regardless of lesion size, morphology, and Helicobacter pylori status. All sensitivities using LCI-AI were higher than 80% whereas those using WLI-AI varied from 64% to 89%.

Conclusion

LCI-AI showed high sensitivity and specificity for the diagnosis of EGCs using all three models tested, suggesting potential for practical clinical use in the near future.

Introduction

Psychiatric comorbidity, most commonly measured as severity of anxiety and depression symptoms, has been found to be associated with IBS symptom severity. However, available studies have rarely assessed the importance of formal psychiatric diagnoses on disease burden in IBS. In this analysis from a pan-European project (DISCOvERIE) focusing on the relevance of comorbid mental (anxiety, depression) and/or somatic (fibromyalgia, chronic fatigue syndrome) diagnoses in patients with IBS, we aimed to assess whether presence of psychiatric diagnoses according to the DSM-5 criteria has an impact on overall symptom severity in IBS.

Aims & Methods

We included patients with IBS according to the Rome IV criteria from nine different European centers. Exclusion criteria included the presence of severe psychiatric disease (i.e. bipolar disorder, schizophrenia, autism spectrum disorder, schizoaffective disorder, or organic psychiatric disorder demanding psychopharmacotherapy, psychiatric day-care or inpatient treatment). The patients underwent The Mini International Neuropsychiatric Interview (MINI) to diagnose major psychiatric diagnoses (a total of 17). Fibromyalgia and chronic fatigue were diagnosed by current diagnostic criteria. They also filled in questionnaires assessing Severity of IBS symptoms (IBS-SSS), depression (PHQ-9), anxiety (GAD-7), somatic symptoms (PHQ-15), and fibromyalgia symptoms (FIQ). Severity of symptoms were compared between groups with increasing number of psychiatric diagnoses using One-Way Between-Groups ANOVA with linear trend analysis.

Results

In total, 617 patients with IBS (median age 44 years, 80% females) were included. Among the IBS patients, 269 (44%) had none of the mental (anxiety, depression) or somatic (fibromyalgia, chronic fatigue syndrome) comorbidities (median age 44 y, 75% females), whereas 174 (28%) had comorbid mental disorder only (median age 38 y, 82% females), 49 (8%) had comorbid somatic disorder only (median age 50 y, 86% females) and 124 patients (20%) had mental and somatic comorbidities (median age 47 y, 86% females). The most common psychiatric diagnoses among the IBS patients were depression (n=284), generalized anxiety disorder (n=145) and panic disorder (n=38) followed by agoraphobia (n=23) and social anxiety (n=19). Participants were divided into four groups according to the number of psychiatric diagnoses (0, 1, 2, and ≥3). The severity of IBS, depression, anxiety, somatic, and fibromyalgia symptoms all increased gradually with increasing number of psychiatric diagnoses. The effect sizes (partial eta squared) were small for IBS symptoms, medium for somatic symptoms and large for the other severity measures (Table 1).

Conclusion

This multicenter pan-European study shows that psychiatric diagnoses are common among IBS patients and that the overall disease burden in IBS is associated with psychiatric comorbidity. These findings highlight the importance of taking psychiatric comorbidity into consideration and to acknowledge the relevance of gut-brain axis interactions when managing patients with IBS.

Introduction

A transmural oxidative imbalance, mainly affecting longitudinal muscle, has been reported in the different clinical phenotypes of Diverticular Disease (DD) (Pallotta L,2022; Cappelletti M,2022) but the genesis of the oxidative stress remains to be defined. Since the central role of the mitochondria for the cell metabolism and radicals production, aim of the study is to elucidate the involvement of mitochondria alterations in the different DD clinical phenotypes in order to improve the knowledge of the DD natural history and management.

Aims & Methods

Longitudinal smooth muscle tissues and cells were isolated separately from surgical specimens of complicated stenotic DD (cDD n=5), up-stream non-occlusive colon cancer with diverticula (DIV n=5) and without diverticula (controls n=5). Oxidized proteins, thioredoxin and, succinate deidrogenase (complex II) and complex IV (cytochrome c oxidase) of mitochondrial respiratory chain were evaluated on tissues by Western Blot. Mitochondrial mass, mitochondrial ROS, mitochondrial membrane potential and glutathione (GSH) were evaluated in cells by FACS. Functional cells activity was tested by image micrometry assessing maximal contraction to acetylcholine and relaxation to VIP, before and after 24h-incubation with 14mM polyphenols (Phenolea Active Complex L). Data (mean±SE) were expressed as fold variations or % decrease from controls, *p value<0.05 considered significant.

Results

In respect to control, oxidized proteins content in longitudinal muscle was increased both in DIV (5.0*±1.1) and cDD (2.6±0.6), associated to a progressive loss of the antioxidant thioredoxin molecule (DIV:70.9%±9.2;cDD:83.3%±3.4). The oxidative status resulted associated to a loss of the mitochondrial complex II succinate deidrogenase codified by nuclear DNA (DIV:43.4%±19.7;cDD:67.9%±10.3) suggestive of the electron respiratory chain impairement. Compensatory stress mechanisms likely occurred with an increase of the complex IV cytochrome c oxidase codified by mitochondrial DNA in muscle tissue (DIV:1.1±0.77;cDD:2.3±1.7) and mitochondrial mass increase (DIV:2.4±1.2;cDD:3.7*±1.3) in cells. Radicals over production was observed mainly in cDD with a cellular increase in mitochondrial ROS (cDD:1.50±0.57) and mitochondrial membrane potential (cDD:1.4±0.3). Antioxidant GSH cellular content progressively decreased from DIV (51.9%*±9.5) to cDD (72.2%*±1.5). These oxidative alterations were associated to an impairment of functional cellular activity with a reduction of contraction (DIV:51.3%*±4.5;cDD:52.5%*±8.5) and relaxation (DIV:34.9%*±14.3;cDD:37.6%*±10.6). The prominent role of oxidative stress in these functional alterations was confirmed by the ability of a 24/h polyphenols treatment to restore GSH content (DIV:1.1±0.06;cDD:1.4±0.3) and relaxation, in both DD phenotypes.

Conclusion

Oxidative stress-driven alterations are present in longitudinal muscle in DD, independently from the clinical stage. In accordance to the severity of the pathology, mitochondrial imbalance resulted more prevalent in complicated diverticular disease. The beneficial effects of the antioxidant treatment could open new tailored-therapeutic and prevention strategies still not tested in this disease.

Clinical Case Summary

A 56-year-old male presented with postprandial vomiting and weight loss beginning three weeks after laparoscopic ulcerorraphy of a perforated pyloric ulcer. An abdominal CT showed marked gastric distension and upper GI endoscopy revealed gastric stasis and a fibrotic stenosis in the duodenal bulb in close relation with a suture stitch, only transposable with an ultra-slim gastroscope. Two sessions of balloon dilation (BD) until 20mm were performed one month apart with poor clinical/endoscopic improvement. Endoscopic ultrasound-guided gastroenterostomy (EUS-GE) was proposed to avoid surgical reintervention.
Using a standard gastroscope a 0.035´´guidewire was passed into the proximal jejunum allowing insertion of a 7-french nasobiliary drain. Jejunal distension was achieved through saline, iodate contrast and methylene blue infusion. The linear echoendoscope was used to identify a distended loop adjacent to the gastric wall and EUS-guided puncture with a 19G needle was accomplished. A 20x10mm lumen-apposing metal stent (LAMS) was deployed in distal gastric body. The flow of blue mixture to the lumen confirmed successful gastroenterostomy creation. The patient was hospitalized overnight and resumed oral feeding the day after the procedure. No symptom recurrence was observed and the gastroenterostomy remained patent during the 6 months of follow-up.
Gastric outlet obstruction (GOO) may result from several malignant and benign causes. Surgical gastroenterostomy, self-expandable metal stents (SEMS) and BD are standard therapeutic options. EUS-GE is emerging as an effective and minimally invasive alternative. EUS-GE presents high technical and clinical success with lower rates of adverse events and reduced need of reintervention when compared to surgery and other endoscopic therapies. In benign GOO, SEMS seems less adequate and BD often results in suboptimal outcomes. The authors exemplify the effectiveness of EUS-GE in a case of GOO associated with a benign condition.

Introduction

Data evaluating the utility of Blue light Imaging (BLI) and Linked Color Imaging (LCI) in the diagnosis of gastric intestinal metaplasia (IM) and atrophic gastritis (AG) is limited.

Aims & Methods

We aimed to evaluate and compare the diagnostic accuracy of BLI and LCI without magnification for the detection of IM and AG.
Single centre prospective study involving consecutive patients aged ≥18 years undergoing diagnostic upper gastrointestinal endoscopy (UGIE), endoscopic follow-up of intestinal metaplasia or gastric cancer screening. Exclusion criteria: previous gastrectomy, contraindication to UGIE due to comorbidities, no gastric mucosal biopsies, refusal of consent. Endoscopy was initially performed with white light image (WLI), followed by BLI and LCI. Standardized photodocumentation of stomach was performed with WLI, BLI and LCI. Imaging features suggestive of IM: lavender color sign and whitish flat elevation with LCI, and light blue crest and white opaque substance with BLI. EGGIM and Kimura-Takamoto Classification (KTC) were applied when IM and AG were suspected. Systematic random biopsies of the antrum, incisura angularis and corpus along with targeted biopsies of suspicious gastric mucosal lesions were obtained. Histology was considered gold standard. Performance of endoscopic imaging in the detection of IM, AG and EGC was evaluated.
Initially, 134 patients were evaluated and 64 were excluded: previous gastrectomy-22, pending histology-20, non-BLI imaging-15, intolerance to UGIE-3, refusal of consent-1, esophageal stenosis-1 and contra-indication to biopsies-2. Final sample included 70 patients.
Qualitative data expressed as percentage were compared with Chi-square test. A p value of <0.05 was considered significant.

Results

Median age was 56 (19-86), with 49 (63%) female. There was family history of gastric cancer in 12(15%) and history of smoking in 35 (44%).
Histological AG was found in 15 (19%) and IM in 20 (26%) patients. WLI was suggestive of IM in 14(20%), confirmed histologically in 9 (64%). IM was suspected by LCI and BLI in 24 (34%) and 23 (32%) cases, confirmed by histology in 16 (67%) and 17 (74%), respectively.
WLI classified 56 (71%) as EGGIM 0 and after LCI and BLI, 10 (18%) and 8 (15%) of these cases had EGGIM 1 or 2. In 10 patients, EGGIM 1-2 with WLI, only 1 was upgraded to EGGIM 3-4 with LCI and BLI. In 3 (4%) EGGIM 3-4 with WLI, after LCI and BLI,1 had extensive IM (EGGIM 6). Among 19(24%) patients EGGIM 1-2 with LCI, 3(16%) had EGGIM 0 with BLI.
Atrophy was suspected by WLI and LCI in 9(12%) and 15(21%) patients, confirmed by histology in 6(67%) and 8(53%) cases, respectively. According to KTC, 5 (6%) were classified as C1, 2 (3%) as C2 and 4 (5%) as O3 with WLI. With LCI images, 8 (10%) were classified as C1, 3(4%) as C2 and 4(5%) as O3. The sensitivity (Sens), specificity (Spec), accuracy, positive predictive value (PPV), negative predictive value (NPV) of WLI, BLI and LCI in detecting AG and IM are shown in table 1.

Conclusion

Both BLI and LCI are promising advanced endoscopic imaging techniques with BLI having higher accuracy than LCI in detection of IM.

Introduction

Dietary and lifestyle recommendations are the cornerstones of the treatment of GERD. However, the evidence of the efficacy of those recommendations is lacking. We performed a retrospective analysis of the GISTAR study¹ participants evaluating the symptoms of GERD and dietary and lifestyle parameters developing a tool for predicting GERD symptoms based on the aforementioned criteria.

Aims & Methods

The aim of the study was to analyse the factors pointing to GERD symptoms through the use of data mining and decision tree classifier.
Methods: GERD diagnosis was evaluated using a self-assessment questionnaire ². Dietary, demographic and lifestyle data were collected within the GISTAR study protocol¹. 3340 40-64 year old participants from the GISTAR study were included in the analysis. 1670 of them did not have GERD symptoms. 41 attributes describing demographic, lifestyle, and dietary habits were used for these respondents to induce a decision tree classification model using Orange Data Mining software³.

Results

Fried potatoes including French fries were the most predictive factor of GERD symptoms (65.3% with symptoms; 1516 from 2321), especially for females (82.6%; 1048/1228). This was more pronounced in the group who consumed sweets once per week (87.2%, 950/1090). In males, who consumed fried potatoes, the next most important factor was the consumption of sweets (46.3%, 431/930).
For participants who didn’t consume fried potatoes (14.6%; 147/1008), 1-3 cups of green tea (37.9%; 66/174) and male gender (71.4 % 20/28) were predictors of GERD symptoms.
The accuracy of the decision tree model was: sensitivity (74.8%), precision (75.6%), specificity (75.9%) and AUC (0.841%).

Conclusion

Fried potatoes, female gender and consumption of sweets are the risk factors predicting the diagnosis of GERD. None of the lifestyle factors were displayed as significant.
