Introduction

Gastric emptying (GE) of liquids and solids is differentially regulated. GE of liquids follows an exponential kinetic and mainly depends on fundic tone, GE of solids requires trituration by antral contractions for passage through the adequately opened pylorus into the duodenum. Adler et al. showed that liquid GE was more often delayed in systemic sclerosis patients compared with GE of solids (Rheumatology 2022), but knowledge on the association between GE of solids and liquids in dyspeptic patients, in general, is limited.

Aims & Methods

To investigate this association and the correlation of GE times with symptoms, we compared results of breath tests using 13C-acetate (13C-ABT, GE-liquids) and 13C-octanoic acid (13C-OBT, GE-solids) in consecutive dyspeptic patients undergoing both tests (Keller et al. UEG J 2021). Normal ranges at our institution for 13C-ABT are 20-35 min (T lag-liquids), for 13C-OBT 50-150 min (T ½-solids). All patients additionally answered the gastroparesis cardinal symptom index questionnaire (GCSI, Revicki APT 2003).

Results

302 patients were included (230 women, age 45.9 (31.7-59.5) yrs, median(IQR)). Median T lag-liquids was 25 (20-35) min, median T ½-solids 78 (60-104) min. T lag-liquids and T ½-solids were weakly associated (R=0.165, p=0.002). Both were indirectly correlated with age (R=-0.280, p<0.0001 and R=-0.179, p=0.0009, resp), and similar in men and women. Concordant results for both tests were achieved in 172 subjects (57.6%): Both were normal in 161 (53.3%), both showed accelerated GE in 10 (3.3%) and both delayed GE in 3 (1%) subjects. Discrepant results were observed in 128 subjects (42.4%), in the vast majority of these, one of the tests was normal, but in 7 subjects (2.3%), GE was accelerated in one test, delayed in the other. 67 patients (22.2%) had delayed GE in at least one of the tests. Only in this subgroup, GE time correlated significantly with GCSI (R=0.242, p=0.024), in particular with the nausea/vomiting subscale (p=0.009).

Conclusion

In a large group of dyspeptic patients, more than 45% had disturbances of liquid and/or solid GE. In less than 5% of the total group both tests were found to be either accelerated or delayed in the same patient. Instead, measurements of liquid and solid GE often differred. In patients with delayed GE in at least one of the tests, severity of gastroparesis symptoms was associated with GE time. Thus, measurements of liquid and solid GE may be complementary and identify patients with different pathomechanism leading to GE disturbances and symptoms.

Introduction

Gastrointestinal angioectasia is a significant etiology of recurrent gastrointestinal bleeding, often requiring hospital care. Octreotide has been proposed as a therapeutic option in patients in whom endoscopic treatment is insufficient, particularly in cases of small bowel or diffuse angioectasia.¹ This study evaluates the clinical efficacy of long-acting intramuscular octreotide in the recurrency of gastrointestinal bleeding from angioectasia.

Aims & Methods

A sample of 21 patients receiving monthly octreotide, either 10 mg or 20 mg, were retrospectively analyzed. Clinical efficacy was assessed by comparing the frequency of hospitalizations, urgency department visits, red blood cell (RBC) transfusions and intravenous iron infusions during the 6-month and 12-month intervals that preceded and followed the start of treatment. Additional contributing factors, including anticoagulation therapy and underlying diseases, were also examined.

Results

The sample studied has an mean age of 78 years, with 13 males and 8 hypocoagulated patients. The Charlson Comorbidity Index averaged 5.6 points, with 5 patients having cirrhosis and 5 patients Osler-Rendu-Weber Syndrome. Hospitalizations significantly decreased after octreotide initiation at both 6 and 12 months of treatment (p < 0.001; p = 0.019). Urgent care visits and RBC transfusions were significantly lower in the first 6 months of treatment (p < 0.001 ; p = 0.007), though this effect was not sustained at 12 months. Intravenous iron requirements did not differ statistically between pre- and post-treatment periods. Patients receiving 10 mg of octreotide had significantly fewer urgent care visits and iron infusions at 6 months of treatment compared to the 20 mg group (p = 0.031; p = 0.026).

Conclusion

In recurrent bleeding from angioectasias, when endoscopic treatment is ineffective, a 12-mounth course of octreotide therapy significantly reduces hospitalizations. Additionally, treatment in the first 6 months proves to be effective in reducing the number of urgency department visits and red blood cell transfusions.

Clinical Case Summary

Splenic granulomas are a rare extraintestinal manifestation of Crohn’s disease (CD). In previously reported cases, the diagnosis was confirmed by biopsy. However, due to the increased risk of complications associated with biopsy, confirmation by imaging is desirable.
52-year-old male patient presented with a previously diagnosed, currently untreated CD.
Symptoms:

• Abdominal pain in the right lower quadrant for several weeks, postprandial
• Bowel movements: 4-6 times/day for a few weeks, liquid consistency
• No fever or weight loss
• Palpation: tenderness in the lower right abdomen

Findings:

• C-reactive protein and leukocytes: not elevated
• Calprotectin: 184ug/g. Stool culture and intestinal cytomegaly virus PCR biopsy negative
• Tuberculosis interferon-g release assay: negative
• Colonoscopy and magnetic resonance enterography (MRE): ileitis of terminal Ileum (Fig. 1)
• MRE: multiple T2w-hyperintense splenic lesions, increased in number since last imaging 2024 (Fig. 2)
• B-mode ultrasound: splenomegaly, round nodules of mixed echogenicity (Fig. 3)
• Contrast-enhanced ultrasound (CEUS): 1,5ml contrasting agent (sulphur hexafluoride microbubbles): arterial isoenhancement and late phase hypoenhancement of nodules (Fig. 3), atypical for haemangiomas
• 18-Fluorodeoxyglucose positron emission tomography with computed tomography: no uptake in the spleen (Fig. 4)

After excluding malignancy, abscesses, sarcoidosis, and infectious causes, the splenic granulomatous lesions represented an extraintestinal manifestation of CD. The patient was started on prednisolone, 60mg/day, resulting in marked clinical improvement in 4 days; with reduced abdominal pain and decreased stool frequency. Patient declined recommended maintenance therapy with ustekinumab.
This CEUS pattern is novel to CD and resembles previously reported enhancement patterns of inflammatory lesions. This is a rare reported case of splenic granulomas as an extraintestinal manifestation of CD diagnosed non-invasively by CEUS.
