Introduction

Endoscopic ultrasonography (EUS) is the most reliable and efficient diagnostic modality for pancreatobiliary disease owing to its high spatial resolution, which allows the detection of small lesions. There is difficulty in making differential diagnosis based on EUS imaging characteristics alone, however, because the majority of lesions are hypoechoic. Assessment of vascularity is another approach that can be used for differential diagnosis. Doppler imaging modalities, such as color-Doppler EUS, power Doppler EUS, and e-FLOW EUS, are used in real-time checking of vascularity. Of these methods, e-FLOW EUS is a directional power Doppler ultrasonography method that has better spatial and temporal resolutions than the others. However, conventional EUS Doppler modes, including e-FLOW EUS, are not suited to visualization of fine vessels and slow flow. Contrast-enhanced EUS (CE-EUS) is better for detecting blood flow and does not have some of these limitations. Assessment of the utility of CE-EUS for detecting intraductal papillary mucinous neoplasms (IPMNs) and gallbladder lesions has been reported. However, the use of contrast agents means that there are some caveats concerning its use, and recent advances in technology have led to the development of detective flow imaging EUS (DFI-EUS) to overcome the problems associated with conventional Doppler mode EUS. Few reports have examined its clinical utility because the method is so new.

Aims & Methods

We evaluate its utility for differential diagnosis of gallbladder lesions and intraductal papillary mucinous neoplasms (IPMNs). Enrolled were patients who underwent DFI-EUS, e-FLOW EUS, and contrast-enhanced EUS for gallbladder lesions or IPMNs. Detection of vessels on DFI-EUS and e-FLOW EUS was compared with that on contrast-enhanced EUS and pathological findings. The vessel pattern was also categorized as regular or irregular. Of the 33 lesions included, final diagnoses were 13 IPMNs and 20 gallbladder lesions.

Results

DFI-EUS was significantly superior to e-FLOW EUS for discriminating between mural nodules and mucous clots and between solid gallbladder lesions and sludge with presence or absence of vessel detection in lesions (P = 0.005). An irregular vessel pattern was a significant predictor of malignant gallbladder lesions (P = 0.002). DFI-EUS is more sensitive than e-FLOW-EUS for vessel detection and differential diagnosis of gallbladder lesions and IPMNs.

Conclusion

Vessel evaluation using DFI-EUS may be a useful and simple method for differentiating between mural nodules and mucous clots in IPMN, between solid gallbladder lesions and sludge, and between malignant and benign gallbladder lesions.

NOVEL BIOLOGICAL AND RADIOLOGICAL MARKERS FOR PREDICTING MALIGNANT PROGRESSION IN INTRADUCTAL PAPILLARY MUCINOUS NEOPLAMS: A PROSPECTIVE STUDY

Introduction

Intraductal papillary mucinous neoplasms (IPMNs) of the pancreas are the most frequently identified precursor lesions of pancreatic cancer (PDAC). Only a minority progress towards malignancy, making their identification a critical point in cancer prevention. The available guidelines are imprecise, commonly resulting in over treatment of indolent lesions.

Aims & Methods

The aim of this study (EMI-IPMN, NCT06706700) is to discover novel biological and radiological biomarkers that can distinguish malignant from benign IPMNs. We focused on Endocrine, Metabolic and Inflammatory (EMI) factors, some pathways that are characterized in PDAC but poorly investigated in IPMN progression.
Patients with a presumed diagnosis of IPMN with indication for surgical resection who underwent high-quality magnetic resonance imaging (MRI) and/or computed tomography (CT), were prospectively enrolled. Peripheral blood (PB) and cyst fluid (CF) samples were collected during surgery, and lesions were histologically classified as IPMN with low-grade dysplasia (LGD), high-grade dysplasia (HGD), or invasive cancer (IC). Malignant IPMNs were defined as those with HGD and IC. Cytokine and hormone levels were measured in both fluids using multiplex assays and compared across the diagnostic groups. Continuous variables were converted into categorical variables based on optimal cut-off values, and both univariate and multivariate logistic regression analyses were conducted.

Results

Among the 205 patients enrolled, 133 (58 females, 75 males, median age 69.2) had a histological diagnosis of IPMN: 49 (36.8%) had LGD, 44 (33.1%) HGD and 40 (30.1%) had IC. Analysis of the EMI profile revealed higher concentrations of several markers (IL-8, VEGF, GLP-1, glucagon, and C-peptide) in CF compared to PB. Specific cytokines such as MCP-1 IP-10, TNFα, and IL-8 were significantly elevated in the CF of HGD/IC cases compared to LGD (p=0.05, 0.041, 0.001 and 0.025 respectively). Similarly, IP-10, IL-8, and MDC levels in PB were higher in HGD/IC patients (p=0.006, 0.008 and 0.004 respectively).
Pancreatic fat content, assessed with mDixon Quant Fat Fraction MRI sequences, was notably higher in HGD/IC patients (13.89%) compared to LGD patients (8.38%, p<0.05), and positively correlated with MCP-1 levels in CF.
Among these biological and radiological markers TNFα in the CF demonstrated a substantial diagnostic accuracy for identifying malignant IPMN, with an area under the ROC curve (AUC) of 0.72. In the final multivariable analysis we demonstrate that high TNFα levels in CF and HbA1c were the two independent risk factors for the identification of malignant IPMN (OR 10.8 and 1.11 respectively).

Conclusion

The progression of IPMN to PDAC is associated with a proinflammatory shift in the tumor microenvironment, reflected by local (CF) and systemic (PB) elevations in key cytokines (TNFα, MCP-1, and IL-8) whose involvement in PDAC progression is well established. Elevated hormone levels in CF, along with the strong association between malignant IPMN and HbA1c, underscore the critical role of endocrine alterations in the early stages of pancreatic tumorigenesis. These findings may be harnessed to enable earlier detection of malignant transformation in IPMN. Moreover, the correlation between imaging markers and biological profiles suggests that MRI could serve as non-invasive proxies for underlying pathological changes. Together, these results offer new insights into the pathophysiology of IPMN progression and may imporve diagnostic strategies

Introduction

Branch-duct intraductal papillary mucinous neoplasms (BD-IPMNs) are frequently associated with acute (AP) or recurrent pancreatitis (RP), likely due to the mucin-induced obstruction of the main pancreatic duct (MPD). According to the 2024 Kyoto Guidelines, pancreatitis represents an indication for surgical evaluation in IPMN patients. However, pancreatic sphincterotomy (PS) may represent a less invasive alternative for patients who are unfit for surgery or as a bridge to surgery, aiming to reduce the risk of further pancreatitis episodes.

Aims & Methods

This single-center retrospective study aimed to assess the efficacy and safety of PS in patients with BD-IPMN and RP. We included 33 patients who underwent PS between 2016 and 2023, with at least 12 months of follow-up. Patients with other causes of RP or prior pancreatic surgery were excluded. The frequency of pancreatitis episodes before and after PS was compared. Clinical success was defined as complete resolution or ≥50% reduction in pancreatitis episodes. Adverse events, presence of worrisome features (WF), and progression to high-risk stigmata (HRS) were evaluated. Statistical analysis was performed using paired t-test and Fisher’s exact test, with p < 0.05 considered significant.

Results

Among the 33 patients analyzed (39.4% female, 60.6% male, mean age 59 ± 13.9 years) clinical success was achieved in 81.8% of the cases. Specifically, 72.7% of the patients experienced no further AP episodes after PS and 9.1% experienced a reduction in episode frequency, while 18.2% of patients did not respond to treatment. Adverse events occurred in 15% of patients, including: 12% with post-ERCP pancreatitis (all mild) and 3% with intra-procedural bleeding; no perforations were reported. During follow-up WF emerged in 5 patients (15%), while no cases of HRS were identified. Among the 6 non-responders, 3 underwent a second PS, with 2 achieving clinical improvement, and 3 underwent duodenocephalopancreasectomy (DCP), including 1 for a concomitant pancreatic ductal adenocarcinoma (PDAC) unrelated to IPMN progression. Notably, no cases of IPMN degeneration were observed in the series

Conclusion

Pancreatic sphincterotomy appears to be a safe and effective approach for managing recurrent pancreatitis in patients with BD-IPMN, potentially reducing the need for surgery in selected cases.

Introduction

Pancreatic cancer has a poor prognosis. Surgical resection is considered the only curative treatment for patients with pancreatic cancer. Improvement of resectability is needed for curative resection, and neoadjuvant chemotherapy (NAC) has been introduced as a promising treatment strategy for pancreatic cancer. Contrast-enhanced harmonic endoscopic ultrasonography (CH-EUS) plays an important role in the diagnosis of pancreatic cancers. CH-EUS for predicting the efficacy of chemotherapy in patients with advanced pancreatic cancer, but there are no reports on the association of contrast-enhancement pattern with pathological response (according to surgical specimens) to chemotherapy.

Aims & Methods

The aim of this study was to evaluate whether CH-EUS is useful for predicting the treatment efficacy of NAC determined by pathological response. Patients who underwent CH-EUS before chemotherapy and surgical resection were divided into two groups according to poor (group-P) or rich tumor vascularity (group-R) determined by enhancement pattern on early- and late-phase CH-EUS. The pathological response to chemotherapy was categorized according to Evans’ classification. Pathological analysis showing tumor cell destruction (>50%) defined a good response.

Results

Early-phase CH-EUS classified 44 patients into group-R and 50 into group-P, whereas late-phase CH-EUS classified 10 into group-R and 84 into group-P. Early-phase CH-EUS classification resulted in significantly higher numbers of patients with a good response in the rich group (n=19) than in the poor group (n=4; P=0.0015). Multivariate analysis showed that assignment to the rich group was the strongest independent factor associated with chemosensitivity (P=0.006, hazard ratio=5.66, 95% confidence interval: 1.17–19.27). In resectable patients, the enhancement pattern was the only independent factor associated with chemosensitivity (group-P vs. group-R, P=0.003; HR [95% CI], 14.59 [1.38–154.38]). Late-phase CH-EUS did not reveal a significant difference between group-P and group-R.

Conclusion

Evaluation of vascular pattern on CH-EUS could be useful for predicting the efficacy of NAC in patients with pancreatic cancer. In particular, the enhancement pattern on CH-EUS could be a necessary feature for determining NAC indications in resectable pancreatic cancer.

Introduction

Endoscopic ultrasound-guided biliary drainage (EUS-BD) is reported to be useful as an alternative treatment for patients who are unable to undergo transpapillary bile duct drainage. EUS-hepaticogastrostomy (HGS), which involves a transgastric approach to the left intrahepatic bile duct, has been suggested as the first-line approach for EUS-BD because of a lower risk of bile leakage in patients with dilated intrahepatic bile ducts. However, there are few reports comparing the effectiveness and safety of EUS-BD procedures between EUS-HGS and EUS-HGS with antegrade stenting (EUS-AS+HGS).

Aims & Methods

Therefore, we compared the efficacy and safety of EUS-AS+HGS and EUS-HGS as drainage methods in EUS-BD. From April 2017 to December 2020, we retrospectively evaluated patients who underwent EUS-AS+HGS or EUS-HGS in our hospital for unresectable malignant biliary obstruction for which transpapillary biliary drainage had failed. We compared the time to recurrence of biliary obstruction (TRBO), overall survival, technical success rate, clinical success rate, adverse event rate, reintervention rate, and length of hospital stay between each procedure.

Results

A total of 68 patients, 22 in the EUS-AS+HGS group and 46 in the EUS-HGS group, were included in the study. In the EUS-HGS group, 38 patients underwent HGS only, while EUS-AS+HGS was attempted in another 8 patients who ultimately underwent EUS-HGS because of unsuccessful guidewire penetration. In the EUS-AS+HGS group, the TRBO was significantly longer, and the re-intervention rate significantly lower than in the EUS-HGS group (both p=0.03). However, there were no significant differences in technical success rate, overall survival, clinical success rate, adverse event rate, and length of hospital stay.

Conclusion

EUS-AS+HGS was superior to EUS-HGS regarding the length of biliary patency and re-intervention rate in patients with unresectable malignant biliary obstruction. The results suggest that EUS-AS+HGS is the preferred treatment when guidewire penetration of the biliary stricture is possible.

Introduction

Intraductal papillary mucinous neoplasms (IPMN) is one of the main pancreatic ductal adenocarcinoma precursors, characterized by a remarkable morphological and spatial heterogeneity. This phenotypical heterogeneity is reflected in IPMN prognosis, with gastric phenotype IPMNs being more often branch-duct and low-grade lesions. There is no biomarker to predict which lesion will progress, complexifying the clinical surveillance scheme. Infiltration of immune cells and fibroblasts in tumor microenvironment (TME) may play a role in this phenotypic heterogeneity.

Aims & Methods

The aim of the present study was to describe the molecular heterogeneity of IPMN lesions at a transcriptional level and their TME. All histological slides from 135 paraffin embedded (FFPE) surgical pancreatic resections for IPMN (2013-2023) were reviewed for selection of regions of morphological interest (ROI) depending on IPMN phenotype and dysplasia. A total of 420 morphologically homogeneous ROI were microdissected for RNA sequencing and 22 from mixed phenotype for DNA sequencing. Additionally, 127 regions adjacent to IPMN lesions were annotated for immunochemistry to quantify immune cell and fibroblast infiltration. Proliferation index was quantified for all microdissections by Ki67 immunostaining.

Results

Among the 135 resections, 49 (36%) were pure gastric, 54 (40%) pure intestinal, 6 (4%) pure pancreatobiliary (PB) and 26 (19%) displayed a mixed phenotype (2 or 3 different phenotypes within the same specimen). Out of the 420 microdissections, 181 were gastric (22% high grade), 195 intestinal (40% high grade) and 44 PB (all high grade). Unsupervised analysis showed an important overlap of gastric and PB phenotype in contrast with intestinal IPMNs. Samples from mixed phenotypes tended to cluster independently and 8 out of 22 showed a common clonal origin. In non-intestinal samples, HG lesions clustered in 2 groups, one purely composed of gastric phenotype and a distinct one composed of gastric and PB. In intestinal samples, HG samples poorly clustered suggesting a biological continuum. Indeed, genes overexpressed in intestinal samples were enriched for cell division and proliferation pathways and the ki67 was significantly higher in intestinal (mean 24% ± 16) compared to gastric samples (mean 6% ± 6.4, p<0.0001). Overexpressed genes in gastric samples were highly enriched in immune response pathways. In fact, TME CD4 and CD8 T cell infiltration was significantly higher in gastric and PB compared to intestinal samples and this, independently of grade of dysplasia. Yet, PB samples displayed a significantly higher TME infiltration of active fibroblasts, regulatory T cells, M2 macrophages and neutrophils compared to gastric and intestinal phenotypes.

Conclusion

Our findings highlight the transcriptomic and microenvironmental heterogeneity of IPMN phenotypes, with distinct molecular programs and immune landscapes associated with each subtype. While intestinal IPMN are characterized by a proliferative, immune-cold profile irrespective of dysplasia grade, gastric and PB lesions show marked immune infiltration, with PB lesions enriched in immunosuppressive cell populations. These results suggest that TME composition may contribute to the differential progression potential of IPMN subtypes and provide a rationale for phenotype-specific biomarkers and interception strategies.

Introduction

Undifferentiated carcinoma (UC) of the pancreas is a rare and aggressive variant of pancreatic cancer. UCs are often concurrent with adenocarcinoma and harbor common genetic features, and pathogenic mutations of KRAS and TP53, and chromosomal polyploidy may associate with the development of UC. However, due to its rarity, the tumorigenesis of UC still needs to be fully elucidated. We aimed to clarify the molecular mechanisms of UC arising from intraductal neoplasms of the pancreas.

Aims & Methods

We performed molecular tracing on 2 cases of UCs derived from intraductal papillary mucinous neoplasms (IPMNs). Multi-regional targeted sequencing using a gene panel targeting pancreatic cancer and IPMN-related 37 genes, copy number variation analysis for KRAS and GNAS using droplet digital PCR, and fluorescence in-situ hybridization (FISH) of KRAS/CEN 12 and MYC/CEN 8 were performed. Tumor phenotypes were evaluated by immunohistochemistry.

Results

Case 1 was an intestinal-type IPMN with UC composed of pleomorphic cells expressing vimentin, ZEB1, and Snail and with mucinous carcinoma. The histological transition was observed each from IPMN. Case 2 was a pancreatobiliary-type IPMN; when it recurred in the remnant pancreas, IPMN was associated with intra-ductal invasive growth of UC composed of spindle cells expressing vimentin and ZEB1 and with an extra-ductal invasion of moderately differentiated tubular adenocarcinoma. Identical somatic mutations; KRAS Q61_E62delinsRK, GNAS R201S, TP53 F113V, and RNF43 S321* in case 1, and KRAS Q61H and TP53 R175H in case 2, and copy number variations; CDKN2A deletion and GNAS and MYC amplification in case 1 and amplifications of KRAS, GNAS, MYC, ATM, and ERBB2 and deletions of CDKN2A and SMAD4 in case 2, were detected in both IPMNs and invasive carcinomas. FISH showed marked polyploidy in the KRAS locus (copy number average 5.1 and 6.1, max 26 and 15, respectively) and amplification of MYC locus (copy number average 6.3 and 5.3, respectively) in UC components in both cases. IPMN components showed intermediate polyploidy of KRAS (copy number average 2.2 and 4.3, max 6 and 7, respectively) and amplification of MYC (copy number average 7.8 and 4.9, respectively). Protein expression of MYC was upregulated in IPMN components, concordant with MYC amplification, while relatively down-regulated in UC components. Interestingly, IPMN revealing papillary nodule showed wild-type allele dominant KRAS imbalance and nuclear accumulation of β-catenin in case 1 and amplifications of GATA6 in case 2.

Conclusion

Increased KRAS burden due to marked polyploidy and mutant-allele specific imbalance, TP53 pathogenic mutation, and MYC amplification were the molecular alterations related to UC derived from IPMNs. Moreover, intermediate polyploidy of KRAS and amplification of MYC had already begun subsequent to TP53 mutation in intraductal components. On the other hand, WNT activation and amplification of GATA6 may be related to intraductal dominant growth and de-selection along with the invasion. This is the first report tracing the molecular alterations associated with tumorigenesis of UC from pre-invasive intraductal neoplasm.
