Introduction

For an examiner to achieve a high-quality screening colonoscopy, a high-quality bowel preparation is essential, according to guidelines 85%-90% of patients undergoing screening colonoscopy should have an adequate bowel preparation. However, this quality varies between patients, a reason for variability might be the level of health literacy.

Aims & Methods

The aim of this study was to analyze whether there is an association between patients’ educational status and adequate bowel preparation in Austria.

Retrospective analysis of data collected within an Austrian national colorectal cancer screening program from 2012 to 2022 was conducted. A logistic regression analysis was carried out, with bowel preparation quality (based on the Aronchick scale) as dependent variable and tertiary education level based on the reported academic degree as independent variable, sex and age were used as control variables. Analyses were restricted to the patient’s first colonoscopy. To account for repeated observations of physicians, the sandwich estimator was employed to obtain robust confidence intervals.

Results

A total of 329,778 patients were included in the analysis. Mean age was 66.76 years, 48.60% were male and 51.40% were female. In total 88.46% (n= 21,833) of patients with a tertiary education level had an adequate bowel preparation versus 84.79% (n=258,641) in group of patients without a tertiary education level. Odds ratio (OR) for academics to have an adequate bowel preparation quality was 1.37 (95% CI [1.33, 1.44]; p<0.001).

Conclusion

In general, our cohort complies with the guidelines in terms of adequate bowel preparation rate. However, with patients having a tertiary education level showing better bowel preparation quality than patients with no tertiary education level, it is important to further improve strategies on how to inform also less educated persons in a more optimal way to enable a better screening colonoscopy quality and to optimize resource use from a clinical as well as a public health perspective.

Pancreatic cystic neoplasms (PCN) are a frequent and clinically challenging condition. PCN prevalence increases with age and reports estimate that they may be present in 2–45% of the general population. In addition, the biological behaviour of the various types of PCN differs (ranging from benign to malignant [table 1]), requiring different surveillance and therapeutic approaches. Correct management of PCN is, therefore, critical for avoiding progression to cancer, but at the same time avoiding unneeded close and long-term follow-up, unnecessary invasive diagnostic procedures and overtreatment.

Abstract

Pancreatic exocrine insufficiency (PEI) is defined as a reduction in pancreatic exocrine secretion below the level that allows the normal digestion of nutrients. Pancreatic disease and surgery are the main causes of PEI. However, other conditions and upper gastrointestinal surgery can also affect the digestive function of the pancreas. PEI can cause symptoms of nutritional malabsorption and deficiencies, which affect the quality of life and increase morbidity and mortality. These guidelines were developed following the United European Gastroenterology framework for the development of high-quality clinical guidelines. After a systematic literature review, the evidence was evaluated according to the Oxford Center for Evidence-Based Medicine and the Grading of Recommendations Assessment, Development, and Evaluation methodology, as appropriate. Statements and comments were developed by the working groups and voted on using the Delphi method. The diagnosis of PEI should be based on a global assessment of symptoms, nutritional status, and a pancreatic secretion test. Pancreatic enzyme replacement therapy (PERT), together with dietary advice and support, are the cornerstones of PEI therapy. PERT is indicated in patients with PEI that is secondary to pancreatic disease, pancreatic surgery, or other metabolic or gastroenterological conditions. Specific recommendations concerning the management of PEI under various clinical conditions are provided based on evidence and expert opinions. This evidence-based guideline summarizes the prevalence, clinical impact, and general diagnostic and therapeutic approaches for PEI, as well as the specifics of PEI in different clinical conditions. Finally, the unmet needs for future research are discussed.

Summary

These recommendations refer to the current management in pancreatic ductal adenocarcinoma (PDAC), a neoplasia characterised by an aggressive course and extremely poor prognosis. The recommendations regard diagnosis, surgical, adjuvant and palliative treatment, with consideration given to endoscopic and surgical methods. A vast majority of the statements are based on data obtained in clinical studies and experts’ recommendations on PDAC management, including the following guidelines: International Association of Pancreatology/European Pancreatic Club (IAP/EPC), American Society of Clinical Oncology (ASCO), European Society for Medical Oncology (ESMO), National Comprehensive Cancer Network (NCCN) and Polish Society of Gastroenterology (PSG) and The National Institute for Health and Care Excellence (NICE). All recommendations were voted on by members of the Working Group of the Polish Pancreatic Club. Results of the voting and brief comments are provided with each recommendation.

Introduction

Pancreatic Ductal Adenocarcinoma (PDAC) is a highly aggressive and lethal malignancy of the pancreas due to limited treatment response and lack of early diagnosis. PDAC accounts for more than 90% of all pancreatic neoplasms and early PDAC may be missed on Computed Tomography (CT) scan due to similar attenuation as healthy pancreas, small size and hidden location.

Aims & Methods

The current study aims at improving detection of PDAC with CT scans using a deep learning model. It uses three different Convolutional Neural Network (CNN) models namely, Visual Geometry Group 19 (VGG19), Residual Neural Network 50 (ResNet-50) and Inception-v3. A compendium of 129,910 abdomen CT images (107 patients) of PDAC patients and 18,942 abdomen CT images (80 patients) of healthy controls were considered from The Cancer Imaging Archive. Additionally, several pre-processing and image augmentation methods were used to prevent overfitting and improve accuracies.

Results

The current model showed high construct validity with markedly lowered computational power. The Inception-v3 model showed the highest accuracy in the detection of PDAC with an accuracy of 97.93%. Specificity of the VGG-19 model was the highest at 98.77 whereas sensitivity was the highest for the Inception-v3 model at 98.75%. Data on the CNN models’ Performance Metrics (accuracy, sensitivity, specificity and area under ROC) were all compared.

Conclusion

The deep learning enabled tool accurately detected PDAC on CT scans versus healthy controls. The models were also reasonably sensitive for tumours below 2 cm.
