Introduction

Tumour necrosis factor-like ligand 1A (TL1A) is a novel therapeutic target for inflammatory bowel disease. Patients (pts) with moderately to severely active ulcerative colitis (UC) treated with the anti-TL1A antibody, RO7790121 (formerly PF-06480605 and RVT-3101), showed significant endoscopic improvement and adequate safety in the phase 2a TUSCANY trial.¹ Here, we report data from the global, randomised, double-blind, placebo-controlled, dose-ranging phase 2b TUSCANY-2 (NCT04090411) trial evaluating the efficacy and safety of subcutaneous (SQ) doses of RO7790121 in adult pts with moderately to severely active UC (total Mayo Score [tMS] ≥6, endoscopic subscore ≥2) who had previously failed conventional/advanced therapy.

Aims & Methods

Pts were randomised to receive RO7790121 SQ 50mg, 150mg, 450mg or placebo (PBO) monthly during the 12-week induction period and RO7790121 SQ 50mg, 150mg or 450mg monthly during the treat-through 40-week maintenance period. Efficacy endpoints were assessed at weeks 14 and 56 in the induction and maintenance intent-to-treat analysis sets, respectively. The primary efficacy endpoint was clinical remission at week 14 by tMS. Safety was assessed throughout the induction and maintenance periods. The secondary endpoint of clinical remission using modified Mayo Score (mMS) is aligned to updated FDA guidance. Other secondary endpoints included clinical remission (by tMS and mMS) at week 56 and endoscopic improvement at weeks 14 and 56. Efficacy analysis by pre-defined biomarker subgroups was also performed.

Results

Overall, 245 pts were randomised and treated; 228 completed the induction and 224 entered the maintenance period of the study. At week 14, 25.5%, 23.3% and 23.9% of pts in the RO7790121 50mg, 150mg, 450mg arms, respectively vs 11.6% in PBO experienced clinical remission by tMS (p>0.05 [Table]). The proportion of pts achieving clinical remission using mMS at week 14 was 29.8%, 35.0% and 31.8% in the RO7790121 50mg, 150mg, 450mg arms, respectively vs 11.6% in PBO (nominal p<0.05). Improvements in clinical remission were sustained throughout the maintenance period (Table). A greater proportion of pts demonstrated endoscopic improvement across all doses of RO7790121 vs PBO at week 14 and week 56. Numerically greater treatment effects were observed in biomarker-positive pts relative to all-comer pts. During the induction period, 47.8% (117/245) of pts reported ≥1 treatment-emergent adverse event (TEAE). The most common TEAEs (occurring in ≥5% pts overall) were anaemia (5.3%) and headache (5.3%). In the induction period, 10 pts experienced serious TEAEs (PBO=4; 50mg=3; 450mg=3); 2 treatment-related (PBO=1; 450mg=1). No pts discontinued the study due to TEAEs. A generally similar safety profile was observed during the maintenance period.

Conclusion

Data from the phase 2b TUSCANY-2 dose-ranging study suggests that RO7790121 has a favourable benefit/risk profile with clinically meaningful improvements in pts with moderately to severely active UC. A phase 3 study will be conducted to confirm these results.

Introduction

Reducing polyp miss rate (PMR) during colonoscopy is imperative in reducing post-colonoscopy colorectal cancer incidence. Artificial Intelligence-based Computer-Aided Detection (CADe) systems are intended to support colonoscopists to reduce PMR. The GI Genius™ intelligent endoscopy module (Medtronic ltd.) is a CADe system, compatible with endoscopy systems from most manufacturers.

Aims & Methods

COLO-DETECT assessed clinical effectiveness of the GI Genius™ intelligent endoscopy module in routine colonoscopy practice by comparing GI Genius™-assisted colonoscopy (GGC) with standard colonoscopy (SC).
COLO-DETECT was a multi-site parallel-arm randomised controlled trial, undertaken in ten English NHS Trusts, amongst adults attending for colonoscopy for the indications of colorectal cancer screening (screening population), or gastrointestinal symptoms / surveillance (symptomatic population). Randomisation was one-to-one, stratified by age category (<60y, 60-73y, ≥74y), sex, colonoscopy indication, and NHS Trust. The trial statistician and co-chief investigators were blinded to allocation.
Primary outcome was mean adenomas per procedure (MAP); key secondary outcome was adenoma detection rate (ADR). Primary analysis was intention-to-treat.
A patient and public involvement representative contributed to all stages of the trial. COLO-DETECT was prospectively registered (ISRCTN: 10451355, ClinicalTrials.gov: NCT04723758, NIHR Portfolio ID: 48022). The protocol was prospectively published.
COLO-DETECT was funded by Medtronic ltd. who had no role in trial design, delivery, or reporting.

Results

2032 participants were recruited between 29/03/21-06/04/23: 28% were <60y, 61·5% were 60-73y, and 10·5% were ≥74y; 55·7% were male; 60·6% were undergoing bowel cancer screening. Randomisation allocation was equal (1015 to GGC, 1017 to SC).
MAP for GGC was 1·56 (SD 2·82) vs 1·12 (SD 1·91) for SC (IRR 1·30; 95% CI 1·15-1·47; p<0·001). ADR was 56·6% (GGC) vs 48·4% (SC), (OR 1·47; 95% CI 1·21-1·78; p<0·001).
GGC demonstrated smaller mean polyp size (5·18mm vs 5·78mm; 95% CI -1·08 – -0·11; p=0·016), more 0-IIa polyps (34% GGC vs 27% SC; ꭓ²(5) = 19·34, p=0·002), and higher sessile serrated polyp (SSP) detection rate (11·6% vs 8·3%; OR 1·49; 95% CI 1·09-2·03; p=0·012).
Within the screening population MAP was 2·08 (SD 3·36) in the GGC group vs 1·61 (SD 2·16) in the SC group (IRR 1·29; 95% CI 1·12-1·48; p<0·001), and ADR was 68% in the GGC group vs 61% in the SC group (OR 1·37; 95% CI 1·07-1·74; p=0·011).
Within the symptomatic population MAP was 0·76 (SD 1·33) in the GGC group vs 0·57 (SD 1·18) in the SC group (IRR 1·33; 95% CI 1·03-1·71; p=0·027) and ADR was 39% in the GGC group vs 28·5% in the SC group (OR 1·65; 95% CI 1·2-2·26; p=0·002).
Advanced adenoma detection rate and cancer detection rate did not differ between GGC and SC groups overall, or within the screening or symptomatic populations separately.
Colonoscopic adverse events were equally distributed between the trial arms with none related to the trial intervention.

Conclusion

GI Genius™ increases MAP and ADR in routine colonoscopy practice, in all patients as well as symptomatic and screening populations separately. Increased detection is of smaller, flat polyps, which are more likely to be missed during colonoscopy. Detection of SSPs was increased, which are over-represented among lesions causing post-colonoscopy colorectal cancer. These increases occurred despite high baseline MAP and ADR in the control arm. COLO-DETECT supports the adoption of GI Genius™ into colonoscopy practice to increase detection of premalignant polyps.
