Introduction

Physical activity (PA) has in adults been inversely associated with risk of inflammatory bowel disease (IBD) however data have mainly been retrospective and liable to reverse causation. Studies on paediatric population are lacking. The aim of this prospective study was to characterize the relationship between PA level in early life and later risk of IBD.

Aims & Methods

The population-based ABIS (All Babies in Southeast Sweden) and MoBa (Norwegian Mother, Father and Child Cohort Study) cohorts follow children from birth (1997-2009) throughout 2021. We retrieved prospectively collected questionnaire data on the degree of PA and screen time (ST) at child’s age of 36 months in both cohorts; in MoBa, similar data were also available at child's age of 18 months. Using linked data from the Swedish National Patient Register and the Norwegian Patient Register we defined IBD as ≥ 2 diagnostic records of the disease. Cox regression was used to estimate hazard ratios (HRs) and 95% confidence intervals (CIs) for IBD, Crohn’s disease (CD), and ulcerative colitis (UC). We adjusted for child’s sex, maternal/parental origin, maternal smoking in pregnancy, immune-mediated co-morbidities, educational level, and parental IBD. Cohort-specific estimates were pooled using a random-effects model.

Results

Overall, we included 65 978 participants in ABIS (n=8 810) and MoBa (n=57 168) with data on PA and/or ST at 36 months of age. During 928 981 person-years of follow-up, 266 participants were diagnosed with IBD (ABIS, n=65; MoBa, n=201). Children categorized into high versus low PA had not significantly increased risk of IBD, CD or UC (adjusted pooled HR 1.12 [95% CI 0.87-1.43], 1.00 [95% CI 0.67-1.48] and 1.23 [95% CI 0.66-2.32], respectively). Similar results were found modelling PA as a continuous variable (Table 1). Also, high versus low ST was not linked to later IBD (adjusted pooled HR 0.91 [95% CI 0.71-1.17]) or its subtypes (adjusted pooled HR 0.98 [95% CI 0.58-1.66], 0.75 [95% CI 0.28-1.98]). Further, comparing children reported to have both low PA and high ST level versus children with high PA and low ST level (Combination PA+ST), we found no association with IBD (Table 1). Among 75 254 MoBa children with 18-months’ data, 256 developed IBD during the follow-up time. In the sub-analysis, none of the above-mentioned exposures (lower versus higher PA and ST, PA and ST analysed as continuous variable and their combination) were significantly associated with a risk of later IBD, CD or UC development (Table 1).

Conclusion

While PA in childhood has shown short- and long-term health benefits, PA in early life was not significantly associated with later risk of IBD in this Scandinavian birth cohort study.

Introduction

Microscopic colitis (MC) refers to two pathologies – Collagenous colitis (CC) and Lymphocytic colitis (LC) that cause chronic watery diarrhoea, urgency, incontinence leading to significantly decreased quality of life¹. It is thought that MC develops due to a pathological immune response to intestinal luminal antigens in genetically predisposed individuals, with microbiota playing an important role^{2, 3}. However, the pathogenesis of MC is still unclear.

Aims & Methods

The aim of this study was to identify and characterize changes in bacterial composition of MC patients in terminal ileum and different colonic segments and determine its relation to the clinical course of disease. Biopsies from terminal ileum and six segments of the colon were collected during diagnostic colonoscopies. DNA extraction was performed using AllPrep DNA/RNA Micro Kit (Qiagen) following the manufacturer’s instructions. To determine bacterial composition, sequencing of the 16S rRNA variable region V3-V4 was conducted using the 27F and 338R polymerase chain reaction primers and sequenced on a MiSeq (2×250 bp; Illumina, Hayward, CA). Bioinformatic analysis was performed using DADA2, and taxonomic classification was carried out with the RDP classifier.

Results

Biopsy samples from 20 active CC patients, 20 active LC patients, and 50 controls demonstrated distinct microbial profiles across different intestinal segments, as revealed by beta-diversity and differential compositional analyses. Compared with controls, CC patients had consistent alterations in three bacterial families throughout the entire colon, with Erysipelatoclostridiaceae and Coriobacteriaceae decreased and Pasteurellaceae increased. Additionally, 35 families had segment-specific changes, and 25 families displayed altered abundance in the terminal ileum.
LC patients exhibited a consistent increase in Erisypelotrichaceae across the entire colon, with 38 other families showing segmental changes, and 12 families displaying altered abundance in the terminal ileum. Comparing LC and CC revealed a significantly different bacterial profile. LC showed lower abundance across many families, particularly in the right colon and terminal ileum. After sample collection, 15 CC patients and 12 LC patients received budesonide, with 3 CC and 2 LC patients having incomplete response. Beta diversity analysis revealed significant differences in the ascending colon and left side of the colon of CC patients based on response to budesonide. Patients with incomplete response to budesonide had an increased abundance of the genus Paucibacter. Similar tendencies were not observed in LC.
1 CC patient exhibited a quiescent disease course, 6 reached a sustained remission after treatment, 7 had a relapsing and 6 had a chronic active disease. Significant differences in beta diversity were observed in transverse, descending colon and rectum. Beta diversity of the descending colon had a medium correlation with disease behaviour. In the LC group 6 patients had a quiescent disease, 8 had a sustained remission after treatment, 3 exhibited a relapsing and 4 had a chronic active disease. No significant correlation between microbiota and disease behaviour was observed in LC.

Conclusion

This study showed significant differences in bacterial composition between CC, LC and controls in the terminal ileum and colon. In both CC and LC, bacterial alterations were not consistent throughout the entire colon but varied between different colonic segments. Microbiome differences based on response to budesonide and disease behaviour were observed in CC, but not LC.

Introduction

The phase 2b GALAXI 1 and phase 3 GALAXI 2&3 studies evaluated guselkumab (GUS), a dual-acting IL-23p19 subunit inhibitor, in participants (pts) with moderately to severely active CD.¹ Pts treated with ustekinumab (UST) who met inadequate response criteria during the long-term extension (LTE) could cross over to GUS 200 mg SC q4w. Here, we present efficacy and safety results in pts who received GUS after experiencing an inadequate response to UST in the pooled GALAXI LTE.

Aims & Methods

Individuals with prior inadequate response or intolerance to UST were excluded from GALAXI; however, during the LTE, pts treated with UST 90 mg SC q8w who met inadequate treatment response criteria (not in clinical response [≥100-point reduction in CDAI score from baseline or CDAI<150] and CDAI ≥220) between Wks 52-80 of LTE were eligible for a treatment switch to GUS 200 mg SC q4w, without IV induction. Clinical response and clinical remission (CDAI<150) were both assessed 16 weeks after treatment switch. Endoscopic response (≥50% improvement from baseline SES-CD or SES-CD≤2) and endoscopic remission (SES-CD≤4 and ≥2-point reduction from baseline and no subscore >1 in any individual component) were assessed at Wk 96. Safety was assessed through Wk 96.

Results

In total, 80 pts treated with UST underwent treatment switch to GUS 200 mg SC q4w maintenance therapy due to meeting inadequate treatment response criteria between Wks 52-80 of the LTE. Of these 80 pts, 75 were included in the efficacy analyses (baseline mean age, 35.2yrs; male, 64.0%; mean CD disease duration, 8.21yrs; mean CDAI, 291.5; mean SES-CD, 12.8; history of inadequate response/intolerance to biologics [BIO-IR], 60.0%).
The proportions of pts achieving clinical response and clinical remission 16 weeks after treatment switch from UST to GUS 200 mg SC q4w were 61.3% and 52.0%, respectively. These results were comparable to the proportions of pts treated with GUS 200 mg IV q4w induction in the BIO-IR subgroup of the total LTE population who achieved clinical response and clinical remission at Wk 12 (61.7% and 46.7%, respectively).
The proportions of pts achieving endoscopic response and endoscopic remission at Wk 96 (ie, ~1 year after treatment switch from UST to GUS 200 mg SC q4w) were 49.3% and 30.7%, respectively. These results were similar to the proportions of pts in the BIO-IR subgroup receiving GUS 200 mg SC q4w maintenance who achieved endoscopic response and endoscopic remission at Wk 48 (57.2% and 35.5%, respectively).

Conclusion

Among pts who experienced inadequate response to UST in the LTE, more than half achieved clinical remission 16 weeks after treatment switch to GUS 200 mg SC q4w, and approximately 50% were in endoscopic response ~1 year after treatment switch. These data suggest pts with an inadequate treatment response to UST may benefit from GUS treatment. Results should be interpreted considering that pts received GUS SC maintenance therapy directly without IV induction. Key safety event rates were consistent with the known safety profile of GUS in approved indications.

Introduction

Many prospective studies conducted in healthy volunteers have shown that diet is associated with the risk of developing IBD. By contrast, data are scarce on the role of diet in disease activity in patients with IBD. The aim of the present study was to find dietary factors associated with the risk of IBD relapses.

Aims & Methods

The “Mikinautes” e-cohort is a nationwide French prospective study recruiting IBD patients older than 6 years at inclusion and followed in private practice and public hospitals. French Crohn and Colitis Patients’ association was also involved in the design, funding and recruitment. Patients filled in online questionnaires about their dietary habits and health status. Validated food frequency questionnaires (FFQ) recorded average intakes of 230 food items over the past 12 months. Patients were followed for at least one year and medical records were prospectively collected. Relapse was defined by i) an increased disease activity reported monthly by the patients and/or a clinical report mentioning a relapse, together with ii) a documented consultation with a doctor and iii) a change in IBD therapy, either drug, enteral nutrition and/or surgery).
A posteriori dietary patterns (DPs) were identified using principal component analysis (PCA) performed on 31 food groups. Primary analysis was a Cox model among patients in remission at inclusion to reduce potential reverse causality. Secondary analyses included: i) a Cox model conducted among all patients, which measured the time between the onset of the last relapse before inclusion (or the ongoing relapse at inclusion) and the first relapse during follow-up and ii) a logistic model comparing inactive versus active patients (defined as patients who had at least one relapse at inclusion or over the first year of follow-up). Models were adjusted for age, sex, smoking status and energy intake.

Results

After exclusion of 14 patients with implausible dietary intakes, 693 patients were retained for analyses, comprising 66% of females, 59% Crohn Disease (CD) patients, 34% ulcerative colitis (UC) patients and 7% indeterminate colitis (IC). Median follow-up [Q1-Q3] was 2.1 years [1.6;2.7]. 77% of patients were in remission at inclusion. Their cumulative probability (95% CI) of flare were 18% [15%; 21%] at 1 year and 25% [22%; 29%] at 2 years. In total, 36% of patients had an active disease after one year of follow-up (including active patients at inclusion).
Four DPs were identified through PCA. In CD, primary Cox analysis revealed an association between the second DP and disease activity (HR 95% CI: 0.42 [0.22;0.80] for 3^rdvs 1^st tertile of consumption, p-trend=0.050). This DP was characterized by high intakes of vegetables, fruit, nuts, legumes, cereals, eggs and unsweetened beverages and by a low consumption of dairy products and desserts, bread, sweet/fat or salty/fat products. Secondary analyses confirmed this association (HR: 0.37 [0.18;0.78] for 3^rdvs 1^st tertile, p-trend=0.010; OR: 0.42 [0.22;0.80] for 3^rdvs 1^st tertile, p-trend=0.010). In UC, fruit consumption was protective toward relapse in primary (HR: 0.46 [0.26;0.82] for 3^rdvs 1^st tertile, p-trend=0.009) together with secondary analyses; no dietary pattern was associated with relapse.

Conclusion

A diet characterized by a high consumption of fruit, vegetables, legumes and nuts is associated with a reduction in the risk of relapse by half in patients with CD. High fruit consumption is associated with the same decreased risk of relapse in patients with UC. These results inform dietary counseling in IBD.

Introduction

Colorectal cancer (CRC) gradually develops from precancerous lesions - colorectal adenomas or serrated polyps - in the gut. As such, removal of these lesions is an effective prevention of CRC [1]. However, currently currently available non-invasive detection methods – such as the fecal immunochemical test – produce a high number of false positive results, resulting in a high number of unnecessary colonoscopes [1]. CRC has been associated with changes in the gut microbiota, but links between microbiome and pre-cancerous lesions are underexplored and it is unclear if gut microbes can predict the future onset of CRC.

Aims & Methods

We linked the Dutch Microbiome Project - a large-scale prospective population study including 8,208 individuals [3] - with the Dutch nationwide pathology database [4] to identify all recorded cases of colonic biopsies in these individuals from the past five decades. This allowed us to associate gut microbiome to detailed histological information about precancerous lesions and CRC, including when were lesions detected relative to fecal sampling as well as lesion subtype, growth pattern, size, location and the grade of dysplasia. We analyzed the function, composition and metagenome-assembled genome strain profiles of gut microbiota in participants who developed (pre)cancerous colorectal lesions before fecal sampling (n=214, years 2000-2015) and patients developing lesions after fecal sampling (n=305, 2015-2022), using n=202 individuals with normal colonoscopy findings and general population as controls. We associated clinical phenotypes (e.g. type and size of lesion) with microbiome composition and function using multivariate regression and permutational analysis of variance (adonis). Furthermore, we explored strain profiles and functions of associated bacterial species by reconstructing their genomes from metagenomic data using metagenomic assembly and binning approaches.

Results

We found 214 participants with colonoscopy identified precancerous colonic developed before fecal sampling (106 cases of low-grade dysplasia, 24 of high-grade dysplasia, 60 of serrated polyps and 26 cases of CRC), and 305 study participants with lesion developed after the fecal sampling (26 cases of CRC, 149 low grade dysplasias, 78 high-grade dysplasias, 52 serrated polyps and 26 cases of CRC). Microbiome diversity was decreased in participants who developed colonic lesions after sampling (FDR < 0.05) and microbiome composition and function were different in patients with pre-existing or future lesions as well as with types of lesions (CRC, high-grade dysplasia, low-grade dysplasia or serrated polyp; adonis FDR < 0.05). Bacterial species from family of Lachnospiraceae, and the genera Roseburia and Eubacterium were linked with future development of lesions, while associated functions involved methanogenesis, biosynthesis of aerobactin, queuosine and flavine and degradation of lysine. Microbiome composition had a moderate predictive power for future lesions and CRC (AUROC ~0.7).

Conclusion

We found that the gut microbiome of study participants who developed (pre)cancerous colonic lesions in 2015-2022 is significantly different from general population, implying that the gut microbiota might be involved in the development of colorectal lesions and cancer. Our results suggest that the gut bacteria might be used to enhance currently used diagnostics for detection of colorectal polyps and that microbiome-modulating therapies might play a role in prevention of colorectal cancer.

Introduction

Guselkumab (GUS), a dual-acting interleukin-23p19 subunit inhibitor, demonstrated efficacy in participants (pts) with ulcerative colitis (UC) treated with intravenous (IV) or subcutaneous (SC) induction. Here, we report the efficacy and safety of SC induction followed by SC maintenance through Week (W) 48 in ASTRO, a phase 3, randomized, double-blind, placebo (PBO)-controlled, treat-through study in pts with moderately to severely active UC.

Aims & Methods

Eligible pts had modified Mayo scores of 5 to 9, rectal bleeding subscore of ≥1, and Mayo endoscopic subscore (MES) ≥2. Pts also had documented inadequate response/intolerance to biologics, Janus kinase (JAK) inhibitors, and/or sphingosine-1-phosphate (S1P) inhibitors (BIO/JAKi/S1Pi-IR) or to corticosteroids, 6-mercaptopurine, or azathioprine. Randomization was stratified by baseline BIO/JAKi/S1Pi-IR status and MES with 418 pts allocated 1:1:1 to GUS 400 mg SC every 4 weeks (q4w) (×3)→GUS 200 mg SC q4w (N=140), GUS 400 mg SC q4w (×3)→GUS 100 mg SC every 8 weeks (q8w) (N=139), or PBO SC (N=139). PBO pts who met rescue criteria were switched to GUS at W16; GUS pts who met rescue criteria stayed on their assigned GUS dose regimen (sham rescue).

Results

At W48, greater proportions of pts in the GUS groups achieved clinical remission compared with PBO (36.7% for GUS 100 mg q8w [Δ 29.5%], 42.9% for GUS 200 mg q4w [Δ 35.6%], 7.2% for PBO) (Table). Endoscopic remission proportions were 25.9% for GUS 100 mg q8w (Δ 20.9%), 26.4% for GUS 200 mg q4w (Δ 21.4%), and 5.0% for PBO. Clinical response, symptomatic remission, endoscopic improvement, histologic improvement, and histologic remission proportions were also greater for GUS vs PBO. Consistent with the results of the overall population, efficacy was also observed for subpopulations of BIO/JAKi/S1Pi-naïve and BIO/JAKi/S1Pi-IR pts. Safety events per 100 pts-years were not greater in the GUS groups compared with the PBO group: adverse events (AE) (GUS 100 mg q8w and 200 mg q4w vs PBO: 308.3 and 310.7 vs 357.1), serious AEs (6.5 and 8.2 vs 38.4), serious infections (0.8 and 2.5 vs 3.7), and AEs leading to treatment discontinuation (4.9 and 4.1 vs 19.8).

Conclusion

GUS SC induction followed by SC maintenance was efficacious through 1 year in pts with moderately to severely active UC, and the safety profile is consistent with GUS in its approved indications including UC.
