Introduction

Preoperative carbohydrate rich drink (CHO) is a component of the Enhanced Recovery After Surgery (ERAS) protocol and has been shown to improve patient discomfort during fasting. Evidence on its efficacy prior to endoscopic procedures such as oesophagogastroduodenoscopy (OGD) is limited.

Aims & Methods

We aimed to investigate whether CHO given prior to OGD can reduce fasting related discomfort and improve overall patient satisfaction. The primary objective was to evaluate if CHO given pre-OGD reduces fasting related discomfort and improves patient satisfaction compared to the other arms. The secondary objectives were to assess the effects of CHO consumed pre-OGD on procedural risks and gastric preparation quality.
This was a single centre, single-blinded randomised controlled trial. Patients undergoing elective outpatient OGD were randomised into 3 arms: CHO vs Water vs Fasting. All patients were instructed to fast 6 hours pre-OGD. Patients in the CHO and water arms consumed CHO (50g Nestle Preload^TM mixed with 400ml water) and 400ml of water only up to 2 hours pre-OGD. Exclusion criteria were: ASA (American Society of Anaesthesiologists), conditions and medication affecting gastrointestinal motility, pregnancy and diabetes. 30 minutes pre-OGD, patients completed a questionnaire consisting of 11 questions on a 9-point Likert scale pertaining to preprocedure discomfort and symptoms such as vomiting, abdominal pain and fatigue. Post-procedure, endoscopists blinded to the study arm, filled up a questionnaire recording complications and gastric preparation quality. One-way ANOVA test was used to compare the mean scores amongst the 3 groups for each question using SPSS version 27.

Results

262 out of 300 patients recruited were analysed. 52.3% were female and the mean age was 54 years. The predominant indication for OGD was dyspepsia. Our results (Figure 1) showed that patients in the CHO arm were significantly less hungry (4.68 vs 5.95; p=0.004) compared to the fasting arm and experienced less abdominal pain prior to OGD (1.74 vs 2.47; p=0.023). There was however no significant difference between the CHO arm vs water for hunger and pain.. Patients in both CHO (4.69 vs 5.86; p= 0.010) and Water arms (4.68 vs 5.86; p= 0.009) were less thirsty compared to those in the fasting arm. (Table 1). We observed a trend in reduction of depression and feeling weak in the CHO arm as compared to the fasting and Water arm, however these results did not reach statistical significance. There were no overall complications. 10.7% of patients had suboptimal gastric preparation with no significant difference amongst the 3 arms. Logistic regression analysis showed that younger age was associated with suboptimal gastric preparation(p=0.043).

Conclusion

CHO consumption pre-OGD reduces hunger, thirst and abdominal pain compared to fasting. Water consumption pre-OGD yielded similar results, with CHO appearing to suppress hunger better. Importantly, all the patients who consumed clear fluids pre-OGD had no significant residual gastric fluid. We propose that allowing consumption of water or CHO up to 2 hours pre-OGD may reduce discomfort during fasting. Larger studies are required to detect significant differences between CHO and Water and overall cost-effectiveness.

Introduction

Endoscopic Vacuum Therapy (EVT) and Fully Covered Metal Stents (FC-SEMS) are standard treatments for anastomotic leaks (AL) following upper gastrointestinal (UGI) surgery^1-3. Recently, a new hybrid device combining an FC-SEMS with a polyurethane EVT sponge (VACStent) has been introduced, showing promising results⁴. However, there is not comparative data between VACStent and traditional endoscopic methods for ALs.

Aims & Methods

In this retrospective study, all patients with AL after UGI oncologic surgery who underwent EVT or VACStent at a tertiary academic center from March 2015 to September 2024 were included. The two endoscopic techniques were compared. The primary outcome was clinical success (CS), with secondary outcomes including number of devices used, treatment duration, and adverse events (AEs).

Results

Over 114 months, 103 patients [92 EVT, 11 VACStent] were enrolled. Of these, 74.8% had AL following Ivor-Lewis esophagectomy. The two cohorts did not differ significantly in terms of comorbidities, neoadjuvant treatment, leak number/median size, or rates of redo-surgery before endoscopic management. Both EVT and VACStent were used as first-line treatments in a similar rate of patients (79.3% vs. 90.9%, p=0.36).
The EVT group had a higher CS rate than the VACStent group, though this difference was not statistically significant (72.8% vs. 54.5%, p=0.21). Compared to the EVT group, the VACStent group required significantly fewer endoscopic sessions (1±0.7 vs. 3±2.5, p<0.001) and had a shorter mean treatment duration (12.1±11.2 vs. 14.1±12.9 days, p=0.62). Additionally, a significantly lower proportion of patients in the VACStent group required admission to the intensive care unit (ICU) (27.3% vs. 84.4%, p<0.001). In the EVT group, three adverse events were recorded (one bleeding episode and two sponge dislodgments), while no AEs were reported in the VACStent group (p=0.54) (Table 1).

Conclusion

In this retrospective study, EVT and VACStent showed comparable success in treating post-UGI surgery leaks. VACStent was associated with a reduced need for ICU admission and fewer endoscopic sessions. Larger comparative data are warranted to define the role of VACStent in the treatment of leaks after UGI surgery.

Introduction

Hepatic encephalopathy (HE) after Transjugular Intrahepatic Portosystemic Shunt (TIPS) affect 35-50% of patients. The use of prophylactic agents (Lactulose and Rifaximin) to prevent post-TIPS HE remains controversial and currently not recommended by professional guidelines. Our aim was to assess real world practice and identify potential predictors for post-TIPS HE and benefit from post-TIPS prophylaxis.

Aims & Methods

Retrospective cohort study of patients undergoing TIPS at the Tel-Aviv Medical Center (2016-2024). The primary outcome was overt HE, with secondary outcomes of liver transplantation (LT) and mortality.

Results

Seventy-nine patients underwent TIPS, median age was 60 years and 52% were male. Primary liver disease was metabolic dysfunction-associated steatohepatitis (MASH) in 34%, viral (20%), and alcohol-related liver disease (ALD) (16.5%). Indications for TIPS were variceal bleeding (52%) and refractory ascites (38%). Prior to the procedure 31% had ≥ 1 episodes of HE, 21.5% were treated with Lactulose and/or Rifaximin, and 58% were on betablocker treatment. At the time of the procedure, the patient's Child-Pugh score was A/B/C in 19%/66%/11% respectively, with a median MELD-Na of 10.9. Median follow up was 22.75 months. Overall, 22 patients died during follow-up and 8 underwent liver transplantation. 45 patients (57%) developed post-TIPS HE, 60% of them during the first three months. Early post-TIPS HE treatment was given to 49%: 10% Lactulose, 5% Rifaximin, and 34% both. Among those patients, 44% were previously (pre-TIPS) on HE treatment. Development of HE was not predicted by pre-or post-TIPS MELD-Na or Child-Pugh score, TIPS diameter, postTIPS gradient, or post-procedure ammonia levels. Rates of HE also did not differ according to initial prophylaxis treatment (59% with treatment, 56% without, p=1). Pre-TIPS AST and albumin were associated with post-TIPS HE (36.6U/L vs. 60.44 U/L, p=0.03; 34.6g/dl vs. 31.8g/dl, p = 0.05). Prior beta-blocker treatment was associated with post-TIPS HE (14% vs 32%, p = 0.01) but not varices as the indication for TIPS. Mortality rate was not affected by the occurrence of post-TIPS HE (28% among those who developed HE vs. 26% among those who didn't, p = 1).

Conclusion

Early post-TIPS preventive treatment was mostly given to patients previously treated with Lactulose/Rifaximin, however it did not affect the rate of post-TIPS HE. Development of HE did not increase mortality.

Clinical Case Summary

Introduction: Rituximab, a monoclonal anti-CD20 antibody, is widely used for treating hematologic malignancies and autoimmune disorders due to its B-cell depleting action. While gastrointestinal side effects like diarrhea are seen in up to 15% of patients, these are typically mild and self-limiting. Severe colitis is a rare but increasingly recognized complication.
Case Description: A 76-year-old diabetic male with a history of ACS (post-PTCA in 2021) presented with 2 months history of generalized abdominal pain, watery diarrhea (6–7 episodes/day), and weight loss. He had a history of follicular lymphoma diagnosed 4 years prior, treated with 6 cycles of CHOP-R followed by rituximab maintenance (375 mg/m² every 3 months). Symptoms began after the 6th maintenance dose.Physical exam revealed mild pallor;abdomen was soft, nontender. Labs showed normocytic anemia and mild neutrophilic leukocytosis. Stool tests were negative for common pathogens, including C. difficile. The patient was planned for a Colonoscopy, which revealed ulcers over the ileocecal valve and rectal erosions. Biopsies showed mixed inflammatory infiltrates, cryptitis, crypt abscesses, and crypt rupture, consistent with severe active ileocolitis. AFB culture and GeneXpert were negative for tuberculosis.The patient was started on oral mesalamine (500 mg TID) along with Rifaximin(400mg) BD for 2 weeks, showing marked clinical improvement.
Discussion: Rituximab-induced colitis is rare and poorly understood. It may result from disruption of mucosal immunoregulation due to B-cell depletion, leading to T-cell-mediated inflammation. Histology may resemble patterns seen in GVHD or autoimmune enteropathy. Most cases resolve with supportive care and discontinuation of the drug; however, some, like this case, may require treatment in the form of 5ASA, or steroids.There is no previous data on the use of rifaximin in such cases. Differentiating drug-induced IBD from primary IBD remains a diagnostic challenge.

There is limited evidence in the literature establishing a definitive association between rituximab therapy and the onset of colitis. However, emerging hypotheses suggest that rituximab-induced depletion of peripheral CD19+ B cells and colonic CD20+ lymphoid aggregates may disrupt mucosal immune homeostasis, thereby contributing to structural alterations in the intestinal architecture and promoting inflammation.
The role of the gut microbiota in the pathogenesis and persistence of mucosal inflammation in inflammatory bowel disease (IBD) provides a rationale for the therapeutic use of antibiotics in selected cases. While systemic antibiotics such as ciprofloxacin and metronidazole have demonstrated clinical efficacy in IBD management, their utility may be limited in drug-induced colitis due to the potential for systemic adverse effects.
In contrast, rifaximin, a non-systemic, presents a favorable safety profile and has shown promise as a therapeutic option in both idiopathic and drug-induced enterocolitis. Its targeted antimicrobial activity within the gastrointestinal tract, combined with minimal systemic exposure, renders it an attractive alternative. However, further prospective, randomized studies are warranted to substantiate its efficacy and long-term safety in this context.

Introduction

Liver elastography is one of the basic diagnostic tools in hepatology practice. The most widely used and most studied tool in the world is transient elastography (Fibroscan), however, this system is unevenly available in Slovakia. Shear-wave elastography is a more accessible method because it is built into standard ultrasound systems. However, the problem of shear-wave elastography remains the interpretation of its results, since the threshold values for significant fibrosis (SF) and advanced fibrosis (AF) are unique for each system.

Aims & Methods

To compare the results of transient elastography (TE) and point shear-wave elastography (pSWE) in a cohort of patients examined simultaneously with both systems and to determine the clinical benefit of pSWE in practice.
We examined all indicated patients over a 15-month period using both TE and pSWE (Hitachi Arietta V70). We recorded liver stiffness values in kPa, and baseline demographic and laboratory parameters. We used TE as the gold standard, with a threshold of >10 kPa for SF and >15 kPa for AF. We evaluated the accuracy of pSWE in predicting the detection of SF and AF. We used three models: the first one containing only pSWE values, and two logit models using pSWE together with common demographic and laboratory parameters. We identified the area under the curve (AUROC) and threshold values for each model and evaluated the probability of SF and AF according to these values.

Results

We examined 500 patients with a median age of 60 (IQR 48-69), male/female 214/286, with a SF rate of 30.8% and AF of 18.4%. The stiffness values according to TE and pSWE showed a highly significant correlation (rho=0.593, P<0.001). We found the accuracy of pSWE alone in predicting SF (AUROC=0.812), and AF (AUROC=0.839) with threshold values of SF>6.34 and AF>8.45 kPa. For values of pSWE<6.34, 6.34-8.45, and >8.45 kPa, the probability of SF was 11.1, 40.9, and 65.4% and AF was 5.0, 14.77, and 48.9%. We created two probability models (for P, range 0 to 1): ASTEL1 (N=369) that included pSWE, GGT, and platelets (AUROC SF=0.856, AUROC AF=0.908), and ASTEL2 (N=467) using pSWE, age, sex, AST/ALT, GGT (AUROC SF=0.857, AUROC AF=0.901). We set a threshold of P<0.33 to exclude SF and P>0.66 to confirm AF. At P<0.33, P=0.33-0.66, and P>0.66, SF was present in 15.6, 62, and 94.2% (ASTEL1) and 13.92, 50.7, and 86.8% (ASTEL2). At P<0.33, P=0.33-0.66 and P>0.66, AF was present in 5.6, 30, and 79.7% (ASTEL1) and in 4.4, 28, 73.68% (ASTEL2).

Conclusion

pSWE alone has an excellent negative predictive value (<6.34 kPa, 88.9-95%) in the diagnosis of SF and AF. Higher accuracy of positive prediction of SF (86.8-94.2%) and AF (73.7-79.7%) can be achieved by using one of two probability models (for P>0.66) combining SWE values with common demographic and laboratory parameters.

Introduction

Endoscopic retrograde cholangiopancreatography (ERCP) is frequently indicated in patients who underwent Roux-en-Y gastric bypass (RYGB) surgery. (1, 2) However, due to the altered anatomy, it is challenging to gain pancreatobiliary access in patients following a RYGB. Several strategies are currently available, but all have their disadvantages. Laparoscopy-assisted ERCP (LA-ERCP), the current golden standard, has higher success rates but also comes with logistical challenges of intraoperative ERCP, peri-operative complications and high costs. (3 - 5) Endoscopic ultrasound directed ERCP (EDGE) is a technique used to create a gastro-gastrostomy by placing a lumen apposing metal stent (LAMS) between the gastric pouch and the excluded stomach, facilitating subsequent ERCP.(6) Although EDGE gained more popularity over the last few years, prospective studies are lacking.

Aims & Methods

The aim of this study is to provide prospective evidence for the efficacy and safety of EDGE, including fistula closure.This multicentre prospective cohort study included all consecutive patients scheduled for elective ERCP after RYGB surgery. EDGE was performed as a two-step procedure. Primary endpoint was overall technical success. Secondary endpoints were technical success of LAMS placement and ERCP individually, persistent fistula and other procedure-related adverse events (AEs).

Results

Between January 2021 and August 2024, 26 patients were included in 4 Dutch hospitals. Overall technical success was achieved in 25/26 patients (96.2%). Median LAMS indwelling time was 14 days [IQR 11 – 28 days]. Two EDGE-related AEs occurred (7.7%); one perforation of the contralateral duodenal wall following scope insertion and one bleeding after LAMS placement. Two ERCP-related AEs occurred (7.7%): one CBD perforation and one post-ERCP pancreatitis. Two patients were lost to follow-up. None of the remaining patients had a persistent fistula, based on radiologic or endoscopic evidence (0/24). No mortality occurred.

Conclusion

This prospective study shows that two-step EDGE is relatively safe and associated with high technical success, without any cases of a persistent fistula. However, AEs occurred in 4 patients (15.4%), of which two were EDGE-related (7.7%).
